For all Viking Camps: If your child will be bringing an inhaler, epi-pen, or other medication to camp, please read the instructions below. All forms must be sent in with camp applications.

If your child will be brining an inhaler to camp, please print and fill out the Viking Sports Camp Asthma Information Form AND the State of Massachusetts Authorization To Administer Medication To A Camper form.

Epi-Pen Information Form

If your child will be brining an epi-pen to camp, please print and fill out the Viking Sports Camp Epi-Pen Information Form AND the State of Massachusetts Authorization To Administer Medication To A Camper Form.

OTHER MEDICATION

If your child will be brining "other" medication to camp, please print and fill out the State of Massachusetts Authorization to Administer Medication To A Camper form.
